If you no longer have the email address you sign in with, answer these two questions. We will then text a code to the mobile number on your record.

The address on the account, even if you can no longer open it. If you have held more than one HPAC account this is how we know which one you mean.
If we do not hold a national insurance number for you, enter your postcode instead.
